Overview
Questionnaires are surveys that your clinic may assign to you as a part of your Care Journey. A questionnaire can range in number of questions and can either be assigned on their own, or as a part of a bundle of other actions, forms, and/or modules your clinic may assign to you.
Completing Questionnaires
Steps:
- Questionnaires will show for you as a task to complete, like other tasks your clinic may have assigned to you.
- When you click on the Questionnaire, a window will open with the contents of the Questionnaire. You can select between multiple choice options for questions, or complete short response questions within this window.
- Once you have answered all of the questions, the “submit” button will be highlighted and you can select it to complete your Questionnaire. After completion, your Questionnaire will now show on your “Done” tab of your portal.
