How Do I Create a New Episode of Care (EOC) for One Patient in a Couple?

Overview

In some cases, you may need to send an assignment to just one member of a couple. For example, when a male partner is completing a sperm freeze. If you don’t have a version of the Form that can be routed directly to the partner, you may consider creating a new Episode of Care for that partner only:

Steps

1. Navigate to the Patient record by searching for the name of the patient you need to assign the form to.
2. Select to the Episodes Tab for the patient.

2. Select the New Episode button in the right corner of the screen
    
3. The new Episode will be created with the selected person as the primary patient.

 4. To add assignments for the patient as a single patient, click the "Add Assignment" button located in the center of the screen.
