Overview
Questionnaires can be created and managed directly in the Staff App. This allows clinics to build and update their own questionnaires without relying on external support.
📌 Note: Configuration of this feature is accessible by Admin users only.
📌 Note: Depending on your clinic's EngagedMD plan, Questionnaires may be limited use. If you are interested in expanding your use of Questionnaires, please reach out to your EngagedMD Representative.
Steps
1. Go to the Staff App Library & Select "Create"
- On the left-hand sidebar, navigate to the "Questionnaires" tab from the "Library."
- In the top-left, select "Create" to begin configuring your questionnaire
➡️ Tip: If Questionnaires are not available to you, they can be turned on for your clinic by contacting your EngagedMD representative.
➡️ Tip: Once submitted, Questionnaire answers are not editable. If a user requests to modify their answers, you will need to send them a new Questionnaire.
2. Questionnaire Options
EngagedMD's new Questionnaire Configuration Platform allows you to drag & drop elements into your questionnaire, or click to add them to your page!
Questionnaire have multiple types of questions for you to choose from:
Short Text: users can respond with a short multi-word answer.
Paragraph: users may enter a fuller multi-sentence response.
Numeric Input: users to enter a number, can be a whole number or contain a decimal.
Date Picker: users can enter a single date. You can opt to display this as a calendar picker, or with drop-downs for specifically picking the day/month/year.
Date Range: users may enter a start & end date of a date range.
Toggle Switch: users indicate "yes" by flipping the switch.
Dropdown Menu: users may select amongst multiple options. Configurators may choose to allow for single selection, multi-selection, or chips (single-select buttons instead of a drop-down menu).
Radio Button: users may choose among multiple choices, selecting a single option.
Single Checkbox: users can check for "yes".
Checklist Group: users may choose among checkbox items as a multi-select.
Signature: users can sign & date their questionnaire. Configurators can choose between allowing users to draw or type their signatures.
3. Configuring your Questionnaire
First, start your configuration by naming your questionnaire at the top of your right window in the "Questionnaire Settings" tab. Then, determine what fields (above) you'd like to add to your page. You can drag & drop fields from your left menu into the form, and use the "six dots" on the right side of the question to re-order them on the page.
Most question fields have similar configuration options:
Question Label: The title for your question.
Description: (optional) You can add a further description of the information you are looking for to assist your users.
Helper Text: (optional) Add in pre-filled text to a field to help users understand what to enter.
Required field: (optional) Toggle this switch to make the question required by the answering user.
Options: Multi-answer questions will provide this field to allow you to configure the answer options.
You may find that you'd like to break up your questions into multiple pages: in the top left of your question picker, you can select "add page" to create a second page for your Questionnaire. Additional pages allow you to split your questions up by topic & can make it easier for users to navigate through larger questionnaires. As you create pages, you may re-order those using the "six-dots" next to each page to meet your needs.
You may also choose your default participants for a questionnaire: under "Questionnaire Settings" select if you'd like by default to have a questionnaire be assigned to a patient, partner, or both participants.
Additionally, you can preview your Questionnaire as your build it: select the "preview" tab on the top right of your screen. To move back into build mode, select the "build" tab next to it.
4. Conditional Logic
📌 Note: EngagedMD is adding enhancements to conditional logic in upcoming releases. Some conditional logic features may not work as expected until this work is complete.
Some questions may need to be hidden or shown to users based on previous answers, or you may want to be able to re-direct user to other resources. Using conditional logic, you can automate actions based on previous patient answers:
At the end of the configuration options for a question look for the conditional logic section and select "add rule". This will automatically turn the question to required: All conditional logic questions must be required.
Then, fill out the logic you'd like. First, you will select your "if" statement and pick your operator and value. Depending on the question type, the operators available and the types of values you can select will differ.
is: the user answer must exactly equal the value entered
is not: the user answer must be anything but the value entered
contains: the user answer must contain the value entered, but could contain more than just that value
is empty: the user answer is empty (no response)
is not empty: the user answer with anything (responded)
Now, determine your "then" statement, or what you want to have happen when your "if" statement is true.
Skip to question: the user will be sent to the question of your choice, skipping other questions on the Questionnaire.
Go to URL: the user will be sent to a different URL that you'd like them to visit, and end the Questionnaire for the user.
End Questionnaire: the Questionnaire will immediately end
Finally, decide what should happen when the conditional logic is not met with your "otherwise" rule. Your options are to "Continue to next" question, "Skip to question" (and select a different question you'd like to send the user to), "Go to URL", and "End questionnaire".
5. Save & Publish the Questionnaire
Once you are ready, select "Save & Close" to save your Questionnaire. You will be brought back to the Questionnaire Library with a pop-up window for details on the Questionnaire you have been working on.
To publish the Questionnaire in the library, click the dropdown next to "Edit" and select "Publish"
📌 Note: Questionnaires can be used to block Forms & Actions in a bundle. The ability to block modules will be added in the future.
6. Viewing Responses
After your questionnaire has been completed by users, you can view your responses.
Go to the Questionnaire Library and select the Questionnaire you are interested in. In the pop-up window, click on "responses" next to the Edit button. You will then be able to view all of the responses to your questionnaire.
Your Questionnaire Activity Results Dashboard will show you an overview of your number of responses & total questions answered, a graph of your responses over time, and a breakdown of your responses by question. You may also customize your view of the data by picking between different display types for each dashboard widget, or adding additional widgets to display data in multiple formats.
Additionally, you can see all answers at the bottom of the dashboard & download your data using the "download" button in the top right corner.
