Overview
Questionnaires are customizable surveys clinics can assign to a patient. A Questionnaire can range in number of questions and can either be assigned on their own, or as a part of a Bundle of other Actions, Forms, and/or Modules a clinic is assigning to a patient as a part of their Care Journey. Questionnaires are configurable by contacting your EngagedMD representative.
📌 Note: Depending on your clinic's EngagedMD plan, Questionnaires may be limited use. If you are interested in expanding your use of Questionnaires, please reach out to your EngagedMD Representative.
Viewing Questionnaires
Steps:
- You can view the contents of a Questionnaire by navigating to your Library, and then selecting the “Questionnaire” tab.
- Within the library, you can search for a Questionnaire in the “search” box.
- Selecting a Questionnaire will allow you to view the contents of the Questionnaire, including the default participants and survey content
- The top right corner of the Questionnaire details also allows you to select “responses”, where you may view the responses submitted for the Questionnaire, and download the responses into a CSV file if needed.
Assigning Questionnaires
Steps:
- Questionnaires can be assigned to a patient following the assignment workflow. Questionnaires can be found within Bundles on the “Bundles” tab, or on their own in the “Questionnaires” tab of the assignment screen.
- You may add as many Questionnaires as you would like during the assignment workflow.
- In the second screen of the assignment process, select the participants and the due date(s) for the Questionnaires, then assign the Questionnaires:
Configuring Questionnaires
Steps:
- If Questionnaires are not available to you, Questionnaires can be turned on for your clinic by contacting your EngagedMD representative to discuss options for upgrading to include Questionnaires.
- Once Questionnaires are enabled, you can work with your EngagedMD representative to discuss your Questionnaire needs & to configure the Questionnaires you need for your clinic.
- If you find that you need updates to your Questionnaires after they are initially configured, your EngagedMD representative can assist you with scheduling any updates or changes needed.
