Overview
Consent Form Markup allows authorized signers to correct or update a consent form after it has already been partially signed, without voiding the form or starting over.
This feature helps clinics:
- Preserve patient progress
- Avoid resending consents
- Maintain a complete audit trail in Sign
- Correct errors efficiently before final approval
By the end of this article, you will understand when Consent Form Markup is used, how it works, and what to expect when a form is marked up.
Steps
A consent form is signed by the patient (and partner, if applicable)
The patient completes their portion of the consent form in Sign.Once submitted, the consent moves forward in the signing order for review
A reviewer identifies an issue
A staff member in the signing order reviews the consent and identifies an error or missing information (for example, an incorrect response or incomplete field).
At this point, the reviewer initiates Consent Form Markup instead of voiding the form.
Markup is initiated in Sign
When markup is initiated:
- The existing consent form is reopened for correction
- Previously entered information remains intact
The form is returned to the appropriate signer(s) for review
The signer reviews and initials the correction
The signer(s) receives a notification that the form was corrected.
They are prompted to:
- Review the updated section(s)
- Initial or re-sign where required
The signer does not need to complete the entire form again.
The signing process continues
Once corrections are completed:
- The consent continues through the remaining signing order
- All signatures are collected as expected
- The form is completed without being reassigned or resent
An audit trail is preserved
Sign automatically records:
- Who initiated the markup
- When the markup occurred
- What changes were made
This ensures compliance and transparency for clinics and patients.
