How do you associate a form with a module?

To increase patient compliance you have the ability to assign a form that is only accesible once an educational module has been completed. Completion of an educational module means that the patient has successfully watched each section of the module along with any knowledge checkpoints (if applicable).

1. Open the patient chart and select the ADD NEW FORM button.

2. Choose the form(s) that need to be assigned and then select the Associated module (optional): dropdown box.

3. You will see a list of modules to choose from. You are only able to associate a form(s) to one module. Once a module is selected, click on the ADD FORM button on the bottom right of the window.

Once the form is added, you will see the form listed under Forms and the status will display In Queue. The name of the associated module will appear next to the form name in parentheses. In the example below, the associated module is Egg Freezing.
